About the role

Our Reconnecting Lives Program provides wraparound outreach support services to St Bart’s consumers moving out of their transitional and supported accommodation and into secure, sustainable housing in the community using a person-centred, housing-led approach.

Based in East Perth, the Case Manager employs a Housing Led approach to facilitate the search for and transition to longer term accommodation.

The Case Manager engages with consumers to develop individual case plans which are person centred, strength’s based and address identified needs and goals, works closely with stakeholders to coordinate holistic services and coordinates capacity building activities which support consumers to develop their independent living skills and prepare consumers to live independently in the community.

Taking into consideration the qualifications, skills and experience of the successful applicant the base salary offered for this position spans from $77,577 to $83,486 per annum plus superannuation.

About you

To be successful in this role you will need a tertiary qualification in Community Services, Social Science, Social Work or similar discipline (minimum Certificate IV) together with relevant experience.

In addition, our ideal candidate has:

  • Experience working with people experiencing mental health or other personal/social challenges and an understanding of Wellness, Recovery and Trauma Informed principles.
  • Well developed communication and interpersonal skills, with the ability to build rapport with consumers, their families, and other networks.
  • Well-developed assessment and planning skills with the ability to adapt to change.

As we work with vulnerable people, the successful candidate will be required to show proof of influenza and COVID-19 vaccinations (unless a medical exemption is provided). The successful candidate must also possess a current First Aid Certificate, a satisfactory National Police Clearance and current ‘C’ Class Drivers Licence.
